Placing an Order Please follow these steps to place an order: If you are interested in The Listening Program but are unsure as to which components you need, please contact Melissa for a free consultation. She will suggest the most appropriate program components for your success. 1. Please fill out the Listening Checklist first (PDF Format) and include a copy with your order. 2. Review the Consent Forms for an adult or child as appropriate. We will send you a written consent form which you will be required to sign and return 3. Print a copy of our order form in Microsoft Word, PDF , or RTF (Rich Text) format. 4. Determine your Shipping Cost using our shipping cost table. 5. Mail or fax your order form to the address shown on the Order Form. Once you order is received we will call you to verify your order and to ensure that the components you have ordered meet your needs. |
